Welcome! I’m D’Neka Bridges, PA-C, a board-certified Physician Associate serving patients since 2017. My experience across primary care, urgent care, occupational medicine, and hospital medicine has shaped one guiding belief: exceptional healthcare begins with listening, understanding, and treating every patient with genuine care and respect.

Unlike many traditional practices, we offer longer appointment times, minimal or no waiting, direct provider access, personalized treatment plans, a strong emphasis on prevention, and a healthcare experience centered around you.
2. What is concierge-style direct primary care?
Concierge-style direct primary care is a personalized healthcare model that provides enhanced access to your provider, longer appointment times, and a stronger focus on prevention and wellness. Our goal is to build lasting relationships while helping you stay healthy—not just treating illness.
3. What services do you provide?
Our services include annual wellness visits, sick visits, chronic disease management, preventive healthcare, weight management, women's and men's health, lab review and interpretation, lifestyle and wellness counseling, telehealth appointments (when appropriate), and DOT Medical Certification Support.
4. Do you perform DOT medical exams?
No. However, we do offer DOT Medical Certification Support for commercial drivers who were unable to pass their DOT medical exam or whose certification has been placed on hold pending medical clearance. We evaluate and treat underlying medical condition(s), then develop a personalized treatment plan to help optimize health and support return to a certified medical examiner for reconsideration when medically appropriate.
5. Who can become a patient?
We welcome Georgia residents who are 18 years and older seeking personalized primary care, preventive healthcare, chronic disease management, weight management, and wellness services.
6. Do you offer telehealth visits?
Yes. Telehealth appointments are available for many conditions for patients located in Georgia when clinically appropriate.
7. How long are appointments?
Most appointments are 30 to 60 minutes, with comprehensive wellness visits lasting approximately 60 minutes. Extended appointment times allow thorough review of concerns, goals, and care planning.
8. How quickly can I get an appointment?
We strive to offer same-day or next-business-day appointments whenever possible.
9. Can I contact my provider between visits?
Yes. Depending on your selected service or membership, you may have direct access to your provider through secure messaging, phone, or other approved communication methods.
10. Do you offer membership plans?
Yes. We offer concierge-style membership options and also offer select services on a fee-for-service basis.
11. How do I become a patient?
Getting started is easy: schedule your consultation, choose the care option that fits your needs, complete new patient paperwork, and begin receiving personalized care.
12. Do you accept walk-ins?
No. Appointments are scheduled by reservation to provide dedicated, unrushed care. Same-day and next-day appointments are available for members based on availability.
13. What conditions do you treat?
We commonly manage high blood pressure, diabetes, high cholesterol, thyroid disorders, weight concerns, minor illnesses, preventive health needs, and many other common adult medical conditions.
14. Do you prescribe medications?
Yes. When medically appropriate, prescriptions are electronically sent to your preferred pharmacy.
15. What if I need lab work, imaging, or a specialist?
We can order laboratory testing, imaging studies, and specialist referrals when medically necessary. We also help coordinate care and review your results with you.
16. Do you accept health insurance?
Vital Bridges is a direct-pay practice and does not bill health insurance for office visits. We can provide documentation you may submit to your insurance company or HSA/FSA administrator when applicable for reimbursement.
17. What if I have insurance?
Many patients use insurance for services outside our practice, such as specialist visits, hospital care, emergency services, imaging, and laboratory testing (depending on insurance plan).
